The flourish, flower and rolodex card were freebies (used with permission)

font of choice (I used a pay font, Grape Nuts ROB)

Open a new canvas 600 x 600 and floodfill with white if you like.

On a new layer copy and paste the rolodex file card.
On a new layer underneath the file card position the flourish as you like.
Stickerize it if you wish.
(select all, float, modify, expand by 4, floodfill a new layer beneath it with white)

Copy and paste the frame and position on the file card
and beneath it on a new layer add your tube
If you have not already, save your work in progress.

Add any background that you wish behind the file card and flourish.
Mine was done by selecting a section of the file card, using it as a pattern
to floodfill a selection I made on a new layer just above the white layer.
Then I added a grid and then used a mask on it. (A grid can be found in VM Toolbox)

Colorize your file card, flourish and background to
match or compliment colors from your tube. Add dropshadows as desired.
Add the flower and pen I supplied if you wish
and colorize and add dropshadows.

All that is left is to add your text, copyright information if needed, and crop and save.
